Ladies and Gentlemen,
Join us for a fun day of golf, plus lunch and a silent
auction, in support of OCC's mission to educate and
support research to find an early detection method and
cure for ovarian cancer.
Basic Information:
* Entry fee: $100 per person, includes green fees, golf cart, lunch, prize ceremony. Make checks payable to “Amgen Inc” and send with entry forms to Angela Vera, M/S 27-4-A
* Suggested minimum $50 donation to OCC is due with registration. Donate online at Active.com. or make a check payable OCC
* Amgen Foundation matches staff donations of $50 and above (ID#21418) (OCC EIN# 95-4840165)
